A New Plan, LLC is currently contracted under Partners in Wellness, LLC. Because of this, all new clients will complete corresponding intake forms.
If you are new, please feel free to fill out these intake forms before our first session. These forms can also be filled out during the first session.

If you feel that you would like to collaborate your session with another provider, simply fill out the following form.
Rates and Insurance
I typically charge $100 per 50 minute session. I do not bill for insurance at this time. However, I do offer a sliding scale.
Payment options include American Express, Cash, Check, Discover, Health Savings Account, Mastercard, and Visa.
Cancellations must be given a minimum of 36 hours prior to session time. Otherwise, you may be charged the full rate for the session.
